Job Description:Deliversquality home health care services to the Agency’s clients under the directionof a Registered Nurse or Therapist in accordance with physician orders, Agencyand industry standards, and federal and state regulations.

Essential Duties and Responsibilities:

•Assists residents with activities of dailyliving, including bathing, dressing, grooming, toileting, eating, transferring,and mobility

•Engages residents in meaningfulconversation, socialization, and activity while providing personal careassistance

•Responds to all resident needs and/orrequests for care promptly, and provides or obtains assistance with care asnecessary

•Observes residents for changes inphysical, emotional, mental, or behavioral condition, unusual symptoms,accidents, injuries, or unusual occurrences, and reports promptly to a licensednurse

•Completes all forms and documentation suchas incident reports, change of condition reports, and behavior tracking, asrequested, and in a timely manner

•Prepares and serves meals, assists witheating, not including tube feeding

•Makes beds, empties garbage, and tidiesrooms daily and as needed

Requirements

Education and Experience:

•Current state CNA license in good standing

•CPR Certificate

•High school diploma or equivalent

•Previous experience working with theelderly in a residential or long-term care setting also preferred
