Personal Care Aide (PCA)
Job Description:Delivers quality hands-on personal careservices to the Agency's clients as established by the Plan of Care (POC) toenable clients to remain in their own homes.
Reports to: RN Supervisor and Staffing Coordinator
Class IV: Paraprofessional Field Staff
F.L.S.A Status: Non-Exempt
Essential Duties andResponsibilities:
·Assists with or providespersonal care as assigned by the R.N. Supervisor.Personal care duties include assistance inappropriate and safe techniques in personal hygiene and grooming:
a. Bathing in bed, tub, shower, or sponge bath.
b. Care of the hair—shampoo in sink, tub, or bath.
c. Shaving and ordinary nail care.
d. Care of the teeth and mouth.
e. Routine skin care, such as applying lotions,excluding any type of products with an “active ingredient,” such as topicalmedications, alcohol, or witch hazel.
f. Toileting—bedpan or urinal, commode, or toilet.
g. Dressing and undressing.
h. Safe transfer techniques, ambulation, andpositioning.
i.Assisting with self-administered medications
·Takes and records vitalsigns if identified on the POC.
·Prepares and servesmeals; assists with eating, not including tube feedings.
·Performs lighthousekeeping duties relevant to the client:
a. Makes and changes bed.
b. Cleans bedroom, bathroom, and rooms used primarilyby the client.
c. Cleans personal laundry in the absence of availableand able family member.
d. Shops for the client in the absence of availableand able family member if identified on the POC.
e. Cleans kitchen and washes dishes after preparingthe client’s meal
·Reports any unusualconditions and/or behavioral changes in the client to the RN Supervisor and/orStaffing Coordinator.Calls StaffingCoordinator immediately when any of the following conditions are identified:
a. Skin breakdown.
b. Falls.
c. Medication changes.
·Uses appropriateprocedures for infection control.
·Provides a safeenvironment for clients; notifies supervisor (Staffing Coordinator and/or RNSupervisor) of any concerns related to maintaining a safe environment forclients.
·Recognizes emergenciesand has knowledge of emergency procedures.
·Completes Personal CareAide records completely, accurately, and timely; writes clearly and legibly.
·Assists in carrying outthe POC as directed by the RN Supervisor as follows:
a.Assistswith simple, non-sterile dressings.
b.PerformsRange of Motion exercises if ordered by a physician and identified in the Planof Care.
c.Measuresand records intake and output.
d.Usesdevices instructed to aid in normal living activities.
e.Teachesself-care hygiene.
·Completes other duties asassigned by designated supervisor.
Education and Experience:
·Be successfully enrolledin a nursing education program preparing for registered nurse or practicalnurse licensure and have currently completed at least one nursing course thatincludes clinical experience involving direct client care.OR
·Has satisfactorily passeda competency evaluation program that meets the criteria of 42 CFR 484.36(b).Home attendants of personal careservices need only be evaluated on the tasks in 42 CFR 484.36 (b), as thosetasks relate to the personal care services to be provided.OR
·Has satisfactorilycompleted training using the “Personal Care Aide Training Curriculum”
·Has physical health andstamina to perform required job tasks.

·Has a minimum of twoacceptable references.
·Must be at least 18 yearsold (regulatory requirement).
·Demonstrates ability toread and write English, to communicate effectively, and to follow instructions.
