SUMMARY
Leads team of nursing and therapy staff and provides nursing services, training, and support to address the nursing and therapy needs for medically complex patients. Provides oversight and coordination of Nursing Intake process, care planning, IDT, and other meetings related to nursing care of the individuals served. As well as, overseeing facility operations.

ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
· Oversees team of CNAs, LPNs and RNs. Evaluates performance and implements corrective actions as needed.
· Directs the quality of nursing assessments, nursing diagnoses, planning, implementation, and evaluation.
· Monitors the nursing delegation process in conjunction with the Assistant Director of Nursing. Ensures required training is completed, reviews delegation decisions, and develops procedures for assigned tasks.
· Conducts the clinical assessment of each individual referred to the program and collaborates with the Assistant Director of Nursing and Intake Coordinator to determine appropriateness for placement in the facility.
· Works with Assistant Director of Nursing to ensure case management requirements are met.
· Provides oversight and coordination of Nursing Intake process, plan of care, IDT, and other meetings related to the nursing care of the medically complex patient.
· Develops and leads medical & clinical trainings in conjunction with the Assistant Director of Nursing, Therapists’, and physicians.
· Directs with expert clinical judgment, based on accepted nursing standards, state administrative codes, contract obligation, policies/procedures and Sevita Quality of Care Standards.
· Evaluates nursing staff in knowledge and application of law, regulation, and policies.
· Conducts on-going quality assurance and improvement audits; develops and implements corrective action plans as needed.
· Coordinates with QI Manager to ensure all QI initiatives are being completed
· Works with Administrative Office Manager to ensure staffing and scheduling meets program needs.
· Alternates Weekend and Evening call with Assistant Director of Nursing.
· Available M-F, 40hrs/week to meet regulatory requirements; unless another RN is available on site.
· May need to flex schedule to meet staffing requirements.
· Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan. Reviews documents and revises all nursing/nursing related care plans on a regular basis.
· Performs other related duties and activities as required.

Minimum Knowledge and Skills required by the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
Education and Experience:
· Graduate of an accredited RN program
· Minimum of five years’ experience working with medically complex patients.
· Preferably experience in Rehabilitation, SubAcute/PostAcute setting.
· One-year supervisory experience required.
Certificates, Licenses, and Registrations:
· Current state RN License. BSN preferred.
· Current driver's license, car registration and auto insurance if providing transportation to individuals receiving services.
· Current CPR/First Aid certification.
Blood Borne Pathogen Category:
· Job requires performance of duties that involve potential for exposure to blood, body fluids, or tissues. Tasks that do involve exposure are an expectation of employment.

Physical Requirements:
· Heavy work. Exerting up to 100 pounds of force rarely, and/or up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently to move objects.
