Inter-Faith Food Shuttle is Hiring a Community Health Educator (Diabetes) Near Raleigh, NC

Part Time up to 15 Hours per week**

Inter-Faith Food Shuttle envisions a hunger free community. We feed our neighbors, teach self-sufficiency,
grow healthy food, and cultivate innovative approaches to end hunger. As a member of Feeding America, the
Food Shuttle distributes over 8 million pounds of food per year, 26% of which is fresh produce. From Senior
Grocery Bags, Backpack Buddies and School Pantries; to Community Health Education and Mobile Markets;
Catering and Culinary Job Training; and Community Gardening and a 14-Acre Farm; we go directly to the point
of need to empower people and overcome the burden of hunger.
The Community Health Educator (Diabetes) is responsible for facilitating monthly teleconferences, providing
individualized support, and creating an instructor guide, lesson plans, and other educational resources. This
position also works with agencies and host site partners in the Food Shuttle seven-county service area to
implement Community Health Education programs efficiently and effectively. This is a grant funded position
funded through April 2025
DUTIES AND RESPONSIBILITIES

  • Creating lesson plans or instructor guide for monthly teleconference centered around Diabetes education.

Teleconferences will be formatted as 3 teleconferences monthly for 1 hour with approximately 25 participants in
attendance.

  • Participate in the development and revision of the Food Shuttle's Diabetes Education instruction materials,

curriculum, handouts, and recipes utilizing evidence-based interventions and current research without offering
medical advice.

  • Demonstrates effective teaching methods appropriate to the patient's age, gender, cultural background,

language, and education.

  • Provides 1:1 education individually tailored to specific participants concerns 2-3x per participant per year.
  • Develop and individualize nutrition care plan with short and long-term goals.
  • Offers knowledge, ability, and skills to support diabetes education to accommodate cultural diversity, age

specific or behavioral needs among the participants being served.

  • Maintains current knowledge of current resources and refers patients to appropriate health care provider or

alternative community resources when indicated.

  • Other duties as assigned.

QUALIFICATIONS

  • Licensure/Certification: Registered Nurse (BSN) or Registered Dietitian (RDN), licensed in NC.
  • Certified Diabetes Educator (CDE) Certification completed or in progress and 3 years experience in diabetes

education and management specifically in hospital, individual counseling, or community base setting OR 4 years
experience in diabetes education and management specifically in hospital, individual counseling, or community
base setting.

  • Current, valid license from North Carolina Board of Nursing or Academy of Nutrition and Dietetics and submit

proof of license renewal every other year.

  • Effective interpersonal skills - oral, written.
  • Experience working with people with lived experience in hunger or food insecurity.
  • Ability to set own schedule, manage multiple priorities and meet deadlines in a fast-paced environment.
  • Proven ability to work steadily and independently with minimum supervision.
  • Ability to work positively with a team.
  • Ability to pass a criminal background check.
  • Excellent communication/public-speaking skills.

WORKING CONDITIONS

  • Work Environment: Work is value, goal and deadline oriented with frequent interaction with community

partners.

  • Primarily computer work in a climate controlled, office setting, with extended hours as needed.
  • Some physical work in both warehouse and outdoor settings, in all weather conditions and temperatures is

required.

  • The noise level is typically moderate.

PHYSICAL REQUIREMENTS

  • Warehousing: requires lifting and carrying crates up to 50 lbs.; walk-in cooler/freezer use
