PACE OF THE TRIAD is Hiring a Home Care Supervisor RN Near Greensboro, NC

Job Summary

The Home Care Supervisor is responsible for the supervision of home care operations and maintaining compliance with all federal, state and PACE regulations governing Home Health Care for PACE Participants. Duties include training and supervision of personal care aide clinical activities, development and implementation of patient care plans.

Specifications

Education and Special Training: Graduate of accredited school of nursing.

Experience: At least one year as an RN, preferably in either home or community health. At least 1 year of experience working with the frail and/or elderly population. At least 1 year of experience supervising a team of at least 4 or more individuals.

Licensure, Registry or Certification Required: Currently licensed as an RN in North Carolina; valid NC driver’s license and vehicle.

Knowledge, Skills, and Abilities: Must be able to work effectively in a team environment. Must be able to treat geriatrics. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid services before working independently.

With or without reasonable accommodations: Ability to bend and/or stoop frequently. Have independent mobility in order to climb stairs and walk distances. Lift and position patients of all weights and heights utilizing correct body mechanics. Lift and carry pieces of durable medical equipment. Lift and transport at least 50 pounds during the course of providing care, treatment and/or services. Utilize correct body mechanics to transfer, lift and position participants. Read, write and speak in English. Communicate effectively both orally and in writing. Ability to clearly see medical records and various documents. Communicate via telephone. Hear blood pressures and spoken communication. Ability to perform repetitive hand/wrist movements.

Key Responsibilities

· Manage home care, including assessments, care plan development, service level determination, on-site field visits, and the implementation, coordination, maintenance and evaluation of care plans.

· Responsible for direct supervision of home care staff to include coaching team members and recommending in-service education programs while ensuring adherence to policies and procedures.

· Ensures quality care services are provided to participants and also participates in care coordination to ensure proper communication between caregivers, participants, and IDT.

· Manages home care operations by determining qualifications, competence and performance expectations of home care team members; discusses operational concerns, updates team members regarding new/changed regulations, and reviews records/documentation to ensure regulatory compliance.

· Actively participates in performance improvement process with team members and the quality department.

· Maintains ongoing clinical knowledge through internal/external training programs to benefit home care operations and staff development.

· Responsible for the implementation and monitoring of staffing plans; conducts orientation and in-service training, as well as oversight of continuing education of assigned team members per guidelines.

· In accordance with guidelines, provides effective leadership by coordinating and integrating services within the home care department and in conjunction with other departments and entities involved in the home care process.

· Assess, using the nursing process, the home care needs of a frail elderly population. Identifies areas of strengths and weakness and develops specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting. Communicates participant changes with team members.

· Coordination of 24 - hour care delivery and the implementation of all home care services to ensure that quality services are provided to meet participant needs.

· Serves as substitute for Health Care Coordinator as described in North Carolina Adult Day Care and Health Services Standards for Certification.

· Actively participates in Interdisciplinary Team (IDT) contributing to the participant plan of care. Communicates any changes in participant condition to team members.

· On-call rotation as assigned by Clinical Coordinator.

· Attends staff meetings.

· Other duties as assigned.

Job Type: Part-time

Pay: $31.00 - $43.00 per hour

Expected hours: 20 per week
