Job Summary

The primary responsibility of the Activity Coordinator / Supervisor is to oversee life enrichment programming, i.e., activities for the PACE program and provide supervision to the life enrichment team. Additionally, the Activity Coordinator assesses the needs, interests and capabilities of participants and develops individualized recreation plans. The Activity Coordinator will plan, organize, direct, and participate in a comprehensive recreation program tailored to the general and individualized needs and limitations of frail elderly program participants. This position is also responsible for the orientation and integration of new participants into the program. The Activity Coordinator works in collaboration with the management team to ensure a high participant and caregiver satisfaction rate, particularly with regard to assigned areas of responsibility.

Qualifications

Education and Experience: Bachelor’s Degree in Recreation Therapy or college degree in related field preferred. Minimum 2 years’ experience in a social or recreational program providing and coordinating services for an aging, frail and/or elderly population within the past 5 years. Two years of supervision strongly preferred.

Licensure, Registry or Certification Required: Cardiopulmonary Resuscitation (CPR)and First Aid certification required. Valid NC driver’s license and vehicle.

Knowledge, Skills, and Abilities:

Must have experience demonstrating strong interpersonal, organizational, group facilitation and consensus-building skills. Possesses the skills necessary to provide care for geriatric patients. Has proven leadership and management skills to include the ability to learn, teach, research, develop and implement policies and procedures and/or projects as needed independently. Possesses the ability to prioritize; multi-task duties within the role, as well as problem-solve and think analytically/critically. Ability to effectively handle difficult and unusual interpersonal situations. Must have experience demonstrating the ability to work in a team environment and foster team cohesiveness.

Job Specific Skills: Working knowledge of recreational therapy techniques and practices of the elderly including assessment skills, care plan development, monitoring, and evaluation needs. Working knowledge of physical, psychosocial, and behavioral needs of the elderly population. Demonstrated creative abilities to plan, develop and implement varied activities to meet diversified needs. Ability to foster high team performance as evidenced by participant satisfaction ratings and key benchmarking data. Ability and experience working with volunteers. Good working knowledge of dementia disease process. Familiarity with illnesses common to frail older adult population. Basic computer skills.

Competency: Position specific competencies for the Activity Coordinator will be met prior to assuming participant care as established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid (CMS) before working independently.

Essential Job Functions:

  • Provides supervision to the life enrichment team to include performance reviews and ongoing training and development for the benefit of program participants.
  • Conducts an in-person initial comprehensive recreational assessment.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person reassessment semi-annually and as needed.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Plans and implements group and individual activities to improve or maintain the participants’ cognitive, physical, social, and psychosocial needs.
  • Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming, and sensory stimulation.
  • Collaboratively develops a monthly calendar of activities. Maintains record of monthly calendars as required for review.
  • Evaluates program plans to promote effectiveness according to abilities and preferences within the ADHC daily flow of events.
  • Provides recreational educational and training to support the life enrichment team, and other PACE Center team members, as appropriate.
  • Maintains participants’ medical record and fulfills EMR charting and reporting requirements.
  • Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
  • Acts within the scope of his or her authority to practice.
  • Maintains general order in the life enrichment area, including daily clean-up, routine supply inventory ordering, and overall general budgeting.
  • Works cooperatively with other staff and departments to meet participants’ needs.
  • Assists participants with non-skilled Activities of Daily Living (ADL) as needed.
  • Protects privacy and maintains confidentiality of all company procedures, results and information regarding team members, participants, and caregivers.
  • Participates in and supports Quality Improvement Initiatives.
  • Participates in DEI-B efforts to support a workforce culture committed to meaningful engagement for all team members, interns, volunteers, vendors, participants, caregivers, and the community at large.
  • Maintains safe working environment. Follows all policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Performs related duties as required.

Compensation:

PACE of the Triad’s compensation system is reviewed annually and adjusted as appropriate after thorough evaluation of market data. All employees are fairly compensated in conformance with specified job qualifications. PACE of the Triad’s compensation structure is consistent with existing legislation in the areas of fair labor standards, wage and hour, equal pay, human rights, employment insurance, and retirement benefits.

The Compensation package includes direct financial compensation, indirect financial compensation, and non-financial compensation.
