The focus of Sadler’s Community Health Workers is to provide support to our patients and their families to establish primary care for health, dental, vision, behavioral health. This elite community outreach team also assists patients in navigation of systems to obtain assistance or resources when needed.

As a Community Health Worker Supervisor, you will be entrusted to lead and manage a team. Overseer your team's time and attendance, scheduling, recruitment, onboarding, training and performance evaluations. You will also compile metric data and evaluate effectiveness of activities and outreach events. 

Your leadership as well as your organizational skills and ability to problem solve and navigate systems and community resources will help you to lead the team to establish, build and maintain trusting working relationships with patients, their families and our community partners in order to assess and identify patient needs and their eligibility for appropriate programs.

As a Community Health Worker Supervisor you will need to be able to work a flexible schedule which at time may include evenings or weekends to attend community events. We also require the ability to travel to and from and work from any of our locations and/or one of our community partners.

We offer a comprehensive compensation and benefits package and pay for this role is between $23-25 per hour.

High School Diploma or GED is required as well as a minimum of three (3) years of experience in a community based healthcare organization or agency WITH at least 6 months of Supervisory /leadership experience.
