Fall River Deaconess Home is seeking a Therapeutic Mentor/Case Manager. The Therapeutic Mentor/Case Manager is responsible for providing direct services to youth and their families. The Mentors support community-based service recipients as they participate in a wide variety of activities designed to prepare them for success in a stable living environment, including practicing pragmatic daily living routines, identifying and engaging in safe recreational activities, maintaining personal boundaries, and learning how to express themselves in a socially appropriate manner. Timely and accurate documentation of these activities is an important aspect of this individual’s role.Are you looking for a way to support youth and their families in the community? If so, this is the job for you!Qualifications:- High school diploma required; Bachelor’s degree is preferred but not required- Experience in an organization which provides services to families, adults and youth- Valid driver’s license- Proven oral and written communication skills- Candidate must be mature, assertive, organized, and able to multi-task and prioritize- Ability to understand, support, communicate with, and work with service recipients and their identified families.- Ability to work collegially with a variety of individuals inside and outside the organization Schedule:Sunday through Thursday OR Tuesday through SaturdaySpecific Responsibilities:- Work collaboratively with youth, family, funding agency and treatment team to identify and address the focal needs of the youth and their family- Provide supportive services within youths’ homes, school and community- Coordinate access to basic needs (food, clothing, shelter and utilities) while providing transportation if necessary- Identify available community-based resources and manage referrals, engagement, follow-up and coordination services- Attend and participate in supervision, staff meetings, trainings, conferences and workshops to promote professional development- Establish positive working relationships with youth and families- Responsible for a caseload working with youth and families- Provide strength-based services utilizing a “wraparound” model- Provide crisis and interventions to youth and families in the home, community, and school- Identify and develop natural supports with the youth and their family- Participate in the creation and implementation of treatment plans- Provide outreach and community-based mentoring which may focus on teaching personal skills related to self-care, social skills, education and pre-vocational attainment and engaging youth in age appropriate social and recreational activities- Maintain record keeping requirements in accordance with agency standards and contractual obligations- Maintain ongoing collaboration with other service providers and funding agencies- Participate in appropriate clinical and administrative staff meetings and in-service trainings- Travel within program catchment area (towns/cities in the southeast region) Benefits:- Fully Paid Dental Insurance – including family plan- Generous contribution to Medical coverage – including paying deductibles- Fully Paid Life Insurance- Paid Vacation Time- Paid Sick Time- Time and a half paid for worked Holidays.- Tuition Assistance after 1 year of Employment- Annual Health Insurance gym membership/weight loss reimbursement- Discounted Healthtrax Fitness and Wellness Center membership- 403(b) Employer Match- Monthly Staff Appreciation RecognitionThis is an ideal position for individuals looking for a starting career in human services and an opportunity for growth.You can visit us at www.deaconesshome.orgFall River Deaconess Home is an Equal Opportunity Employer (EOE)
