Region VII Area Agency on Aging is Hiring a Long Term Care Ombudsman Near Bay, MI

BRIEF SUMMARY OF PURPOSE OF POSITION:
The LTC Ombudsman visits all long-term care facilities in Region VII’s ten county service area on a regular basis advocating for long term care consumers residing in nursing homes, homes for the aged, and adult foster care homes. Visits long-term care facilities in response to complaints. Educate the public regarding all aspects of the long-term care system. Duties also include advocacy, community education, assisting individuals in accessing long-term care services, assisting in development of resident/family councils, and supporting volunteer advocates. Participates in agency philanthropic activities.
RESPONSIBILITES AND DUTIES (not an all inclusive list):
  • Routinely visit residents of long-term care facilities and ensure timely access to representatives of the Ombudsman program.
  • Assist long-term care facility residents in understanding their rights.
  • Assist individuals seeking placement in long-term care facilities in accessing needed services
  • Represent interests of residents before government agencies and seek administrative, legal and other remedies to protect health, safety, welfare and rights of residents.
  • Establish and maintain relationships with public and private guardians, state and local law enforcement agencies, legal assistance programs, courts of competent jurisdictions and other organizations providing support or assistance to long-term care facility residents.
  • Crisis intervention by responding promptly and directly to crisis situations as they occur.
  • Maintain confidentiality in accordance with State and Federal policies.
  • Working with Wavier Providers for identifying Nursing Facility Transitions (NFTI).
  • Assist older adults and their families in understanding, identifying, locating, evaluating, and/or obtaining long-term care services also linking this population to necessary and wanted community resources once LTC residents.
  • Recruit, train, supervise and otherwise support activities related to volunteer advocates assigned to assist residents of identified long-term care services.
  • Ensure compliance with all State and Federal service standards, program guidelines and reporting requirements.
QUALIFICATIONS AND SKILLS:
  • Certification by the State Long Term Care Ombudsman as a Local Long Term Ombudsman or the qualifications and willingness to obtain certification.
  • Must have reliable transportation and valid driver’s license.
  • Bachelor’s degree in Social Work, Gerontology or related field with a minimum of two years experience in human services serving aging or disabled people, required, or comparable combination of education, experience and training.
  • Legal background helpful.
  • Two years experience providing client advocacy, elder abuse and recipient rights counseling to older or disability adults is preferred.
