Job Description:

The Home Health Case Manager is responsible for overseeing and coordinating the care of patients receiving home health services. They work closely with healthcare providers, patients, and their families to ensure that care plans are implemented effectively, and patient needs are met.

Key Responsibilities:

- Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.

- Develop individualized care plans in collaboration with the patient, family members, and healthcare providers.

- Identify specific goals, interventions, and timelines for achieving desired outcomes.

Coordination of Care:

- Coordinate and schedule healthcare services, including nursing visits, therapy sessions, and medical appointments, based on patient needs and care plans.

- Facilitate communication among members of the healthcare team to ensure continuity of care.

-Collaborate with other healthcare professionals, such as physicians, therapists, and social workers, to address complex patient needs.

Requirements:
