Description

Perform general social work services that endeavor to maintain, strengthen and safeguard home and family life where there are social and/or health needs that interfere with independent functioning of individuals or families. Provide services to people regardless of their ability to pay, within the resources available.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Provide rehabilitative and supportive casework geared toward restoring patients to their optimum level of social and health adjustment including assisting patients and their families in understanding, accepting, and following medical recommendations.
  • Help patients utilize the resources of their families and the community including referring patients to resources or acting as an advocate on behalf of the patients in their dealings with other health and welfare agencies.
  • Assist patients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care to include counseling members of the patient’s family to assist patients with admission to a nursing home.
  • Consult with the physician and other members of the healthcare team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems.
  • Prepare clinical/progress notes on the day of the visit and incorporate same in the clinical record and provide summaries and re-evaluations as applicable.
  • Attend case conferences and IDT.
  • Participate in staff development and revision of the physician’s plan of treatment.
  • Participate in staff development activities and in-service education.
  • Participate in discharge planning and in-service programs.
  • Act as a consultant to other agency personnel.
  • Communicate effectively with all healthcare providers.
  • Confirm, on a weekly basis, the scheduling of visits with the Case Manager to coordinate necessary visits with other personnel.
  • Provide notifications that affect agreed services with the company.
  • Provide services on a short-term basis (two to three visits) to a beneficiary's family member or caregiver when it is shown that a brief intervention is necessary to remove a clear and direct impediment to the effective treatment of the beneficiary's medical condition or to the rate of recovery.
  • Take appropriate action to obtain available community resources to assist in resolving the beneficiary's problem.
  • Perform other duties assigned.

Requirements

EDUCATION AND EXPERIENCE

  • Master’s degree from an accredited school of social work approved by the Council of Social Work Education.
  • Current MSW license to practice Social Work in the State of New Hampshire is required.
  • Minimum of 1-year experience as a Social Worker, preferably in a home healthcare setting.

SKILLS AND ABILITIES

  • Ability to assess social and emotional factors related to the beneficiary's illness, the need for care, their response to treatment, and adjustment to care.
  • Ability to assess the relationship of the medical and nursing requirements to the home situation, financial resources, and the community resources available.
  • Ability to work independently and take initiative.
  • Good judgment and problem-solving skills.
  • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
  • Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
  • Ability to document patient care accurately according to healthcare standards and plan of care.
  • Working knowledge of Microsoft Office software and other industry specific programs.
  • Must demonstrate ethical conduct and build trust with patients and family members.
  • Ability to follow safety guidelines as instructed by the agency.
  • Ability to wear protective clothing such as mask, glove, head cover, safety glasses, etc.
  • Ability to travel required. Travel throughout central New Hampshire is required.
  • Current drivers’ license is required.
  • Proof of automobile insurance is required.
  • Ability to pass a motor vehicle background screening is required.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, fingering, grasping, feeling, talking and hearing.
  • Job requires light physical work including the ability to exert up to 20 lbs. of force occasionally, 10 lbs. regularly.
  • Job requires close visual acuity including the ability to see at or within arm’s reach.
  • Job is subject to inside environmental conditions; low noise level.
