Applewood Centers is a premier provider of community-based behavioral health services in Northeast Ohio. We are currently hiring a Clinical Therapist to serve youth (age 2-20) and families who call the crisis hotline with a family identified emergency.
Position Summary:
The Licensed Case Manager provides intensive home and community based skill building and case management services to emotionally and behaviorally troubled youth and their families. As the Licensed Case Manager, you will be responsible for maintaining a caseload of youth and families, engaging clients and families as partners in the care planning process. Caseload size may vary based upon the needs of assigned families.
Responsibilities Include:
Interview clients to obtain information pertinent to establishing and maintaining effective client records and service plans (i.e., PDE, ITP). Gathers information in a timely and systemic manner that will enable ongoing diagnosis, assessment, and treatment planning.
Demonstrate the ability to develop and maintain strengths-focused, professional alignment and engagement with families.
Develop focused, measurable, and achievable service plans that are consistent with the family's identified needs and cultural assessment.
Assists and advocates on behalf of clients and families to secure needed resources, services, and supports.
Collaborate with families and clients in the development of crisis plans and provides crisis assistance and coaching as needed.
Assist clients and families with the development of skills that will enhance and improve functioning in areas that include, but are not limited to, interpersonal relationships, anger management, and coping.
Qualifications:
Minimum Bachelor's level Social Worker (BSW) or Masters degree in Social Work, Counseling or MFT.
Professional licensure in the State of Ohio in social work, counseling or marriage and family therapy (BSW/LSW or LPC or MFT)
A valid driver license, personal transportation, and auto insurance.
