SummaryUnder the supervision of the Nutrition Services Assistant Coordinator, provides Medical Nutrition Therapy, nutrition counseling, insulin instruction, self-blood glucose monitoring, community outreach and education, appropriate referral and follow up for patients and families based on identified high-risk condition and individual need. Work involves direct patient contact and is performed in accordance with agency and funder requirements.Scheduled Hours: 40 hours per week
  • Two days per week on-site and option for remote work 3 days per week.
  • One Saturday per month
• Achieve patient visit and program goals.
• Provides medical nutrition therapy, patient centered nutrition counseling using motivational interviewing, and appropriate referral for patients according to established criteria.
• Educates patients on diabetes self-management and collaborates with patient’s provider on the selection and initiation of pharmacotherapy, instructing on self-administration, use of various delivery devices for injectable medications, recommending dose adjustments, and evaluating medication administration techniques.
• Documents clinical information including nutrition care plans (progress in improving health and documentation of outcomes) in patients’ chart according to agency standards.
• Assists in implementation and monitoring of protocols, policies and procedures to ensure services meet program/grant objectives and are in compliance with agency/funder requirements.
• Participates in ongoing program evaluation including monitoring of program/grant performance measures and implementation of Continuous Quality Improvement activities to improve program and health outcomes for clients/patients.
• Demonstrates support of service and program area philosophy and decisions in verbal and written communication with supervisors, staff, patients, and community.
• Develops and presents nutrition education/inservice training and outreach activities for patients, staff, and the community.
• Provides orientation and training for staff and dietetic interns.
• Assists in the development, standardization and revision of appropriate patient education handouts, outreach materials and referral resources.
• Participates in mandatory in-services and drills, attends staff trainings and other continuing education appropriate for professional develop and job duties.
• Assists and encourages patients in identifying self-management goals.
• Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce Health Disparities within our communities.
• Develops and maintains a basic knowledge of services available at the LCHD/CHC.
• Demonstrates adherence to National Patient Safety Goals.
• Develops and maintains a working knowledge of individual Emergency Preparedness roles.
• Reports all unusual occurrences that may involve the safety and well being of the clients and co-workers, directly to supervisor and/or Safety Officer.
• Performs duties in a prioritized, organized, and orderly manner to maximize clinic efficiency and productivity.
• Maintains orderliness and cleanliness of clinic and work area.
• Performs support staff functions and other duties as necessary.• Requires a Bachelor’s Degree in Nutrition related field.
• Minimum of two years clinical/community experience (one year if Master’s Degree).
• Requires current Illinois licensure as a Licensed Dietitian.
• Requires current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.
• Requires current certification as a Certified Diabetes Care and Education Specialist (CDCES).
• Ability to work on evenings, weekends, holidays and overtime if necessary.
