Job Description: MedZed Physician Services utilizes telemedicine technology to bring healthcare to patients with complex medical, behavioral, and social needs. Our practice includes Primary Care Providers (MD, NP, and PA), Behavioral Health Specialists, Palliative Care Team, field nurses, community health workers and care coordinators. Our nurses visit patients in their homes and use telemedicine to connect back to the PCPs over our novel technology platform. This allows our PCPs to round on complex patients in their homes without having to drive. Our community health workers and care coordination team manage authorizations, social services, and other patient needs. We partner with health plans and other managed care organizations to help high-risk patients stay healthy and out of the ER. We are currently recruiting compassionate Primary Care Providers to treat patients remotely over our telemedicine platform in a team-based model of care. You will manage an adult patient panel, attending to chronic, acute, preventive, and sometimes palliative care needs. You will collaborate with a team of nurses in the field who visit your patients, teach patients self-management skills, and operate the telemedicine platform as you interact with and examine the patient. Our team takes care of scheduling, coordinating authorizations and social services, records retrieval, telephonic patient check-ins between visits, and billingso you can focus on taking care of the patients. We provide all infrastructure and administrative support, telemedicine training, EHR, and patient recruitment Qualifications Job Requirements Applicants for this position must have an active Illinois license in good standing and be Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics.

Strong clinical skills with at least 3 years of practice
Experience providing longitudinal care for a panel of complex patients.
Compassion for patients with complex medical and social needs
Comfort collaborating with nurses in the care process.
Flexible and able to innovate a novel care model.
Can-do attitude, good analytical person, and well organized
Comfortable with technology, including EHR and videoconferencing/telemedicine.
Spanish language skills preferred.
Active Illinois License (NP/PA or MD) in good standing required
For MD candidatesBoard Certification/Board Eligible in Internal Medicine, Family Medicine, or Geriatrics, required.
