POSITION SUMMARY
At Home Assist Health, our Direct Support Professionals (DSP) are instrumental in providing quality, independent lifestyles for people living with disabilities, and ensuring that they have the best care experience possible.
Under the direction of the DDD Supervisor, this position provides assistance to meet the non-medical needs of members in their homes. Direct Support Professionals assist members with personal care activities: routine house cleaning, laundry, ambulation, transfers, meal preparation, assistance with feeding, companionship, and assistance with learning objectives as described in the service plan. This position requires traveling between members’ homes. The employee must have reliable, insured transportation or the ability to utilize the transit system to ensure clients scheduled hours of care are covered. Working hours will vary and may require evening, weekend and holiday hours.

ESSENTIAL FUNCTIONS
· Appropriately assisting member with personal care activities, routine house cleaning, laundry, ambulation and transfers, meal preparation and feeding, and learning objectives in the service plan.
· Provides general supervision of the member and companionship in accordance with the member’s service plan as outlined by the assigned Member Care Coordinator.
· Coordinates shopping and accompany clients on errands, doctor/clinic appointments and assist with arranging transportation as needed and/or as approved.
· Implements community learning objectives as outlined in the service plan.
· Accurately document services provided and complete data sheets/electronic visit verification in accordance with policy and service plan requirements on day that services are rendered.
· Immediately report any changes in client’s condition or incidents to the assigned DDD Supervisor or after-hours Program Coordinator
· Assignments are fulfilled with reliability and punctuality utilizing personal conduct guidelines outlined in the Member/Caregiver Agreement.
· Maintain confidentiality regarding all personal client information.
· Maintain training/certifications as required.
· Maintain a professional appearance and adhere to dress code.
· Conform to ongoing background/fingerprint/attestation/affidavits as required.
· Performs other duties as assigned.
COMPETENCIES
· Requires the ability to read, write and speak effectively in English. If unable, must provide interpreter for interview, physical, and training classes.
· Good communication skills and the ability to follow directions, policies and procedures.
· Minimum of three months experience providing personal care and housekeeping, prefer one year of experience as a Caregiver.
· Additional experience or training may be required for specific services.
· Must be at least 18 years of age.
· Bilingual a plus.
TRAINING AND EXPERIENCE
· AHCCCS Direct Care Worker Training or current AZBN CNA/LNA.
· DCW Developmental Disability training module.
· Skill Building/ISP Process or Habilitation Training.
· Article 9 Training.
· Prevention & Support Training required for specific services.
· CPR and First Aid Certification.
PHYSICAL REQUIREMENTS
· Must be able to lift 25 lbs.
· May require sitting or standing for long periods of time
