Summary of Benefits:

Full-time team members are eligible for Prestige’s comprehensive benefit package including choice of medical plans, HSA/FSA accounts, dental and vision insurance, and company paid life and AD&D insurance. Full-time and Part-Time team members (over 20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.

POSITION SUMMARY

The Senior Social Services Director 1 directs and oversees the social services program in a skilled nursing center to assure that the medically related emotional and social needs of the resident are met and maintained in accordance with all applicable laws, regulations, and Prestige Care standards. In addition to performing the duties of a Social Services Director 1, the Senior Social Services Director 1 provides social services support to assigned center(s), trains, and mentors other Social Services Directors, and supports company-wide social services projects and initiatives.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Meets with all new admissions to document the resident's psychosocial history and identify the current psychosocial needs.
  2. Ensures comprehensive psychosocial care plans are in place for each resident and are completed within the required timeframe and updated quarterly and as needed.
  3. Collaborates with the leadership team, the resident and/or resident’s representatives, and physician to determine discharge plans. Communicates regularly with all parties regarding the plan and encourages resident and/or resident’s representative participation in all steps of the planning and discharge process.
  4. Coordinates home health, hospice, and support services, including making referrals and obtaining services from outside agencies as needed.
  5. Arranges ancillary services including but not limited to dental, ophthalmology, and audiology services. May be required to coordinate appointments for such services and arrange transportation.
  6. Completes and issues Notice of Medicare Non-Coverage (NOMNC) and Skilled Nursing Facility Advance Beneficiary Notice (SNFABN) as applicable.
  7. Completes assigned sections on the MDS and participates in the coordination of Medicaid services.
  8. Documents care plan and services provided in the resident’s record in accordance with state and federal regulatory requirements and Prestige policies. Ensures that all entries are accurate, timely, and descriptive of the services provided.
  9. Acts as a resident advocate by ensuring that services are provided in the resident’s best interest and educates on residents’ rights.
  10. Acts as the grievance officer and coordinates the center’s grievance process.
  11. Assists residents in accessing mental health services and other community resources.
  12. Arranges and obtains needed durable medical equipment, clothing, and personal items for residents.
  13. Conducts and assists in coordinating resident and family support groups including the development of programs for special needs residents.
  14. Manages behavioral health services including tracking, meetings, care planning, care and services. Coordinates and updates Behaviors Monitors for residents as required.
  15. Partners with the clinical team in training staff on resident’s behaviors and appropriate behavioral interventions.
  16. Participates in leadership team by actively contributing to decision making, grievance follow up, building and company-wide initiatives and attending leadership team meetings such as Stand-up, Managing Acute Care Conditions (MACC), Quality Assurance Performance Improvement (QAPI) and other meetings as required.
  17. Provides social services support to assigned center(s) by partnering with regional and clinical leadership team to review social services processes, assisting in problem solving and developing a discharge plan for challenging cases, to review compliance with social services systems, and providing general social services oversight through routine communication and potential site visits.
  18. Assists with the onboarding and development of assigned Social Services Director(s) through coaching, mentorship, and training. Provides on-call support to assigned Social Services Director(s) by answering questions and providing guidance.
  19. Reports on assigned center(s) and Social Services Director(s) to regional leadership team.
  20. Collaborates with the regional and clinical leadership team to support implementation of strategic social services goals and objectives. Participates on special projects and work groups as requested.
  21. Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

Must hold appropriate state licensure if required under state regulations. Three years’ experience working in skilled nursing or similar health care setting or ability to demonstrate equivalent knowledge and skills. Knowledgeable of the rules, regulations and guidelines that govern nursing care facilities.

EDUCATION

Bachelor’s degree in social work, human services or related field required. Master’s degree preferred.
