We are currently looking to hire an Registered Nurse to join us as a Hospice Intake Coordinator, based at our office in West Orange, NJ. The position would be responsible for providing assessment of patients hospice needs and coordination of care for hospice patients.

Responsibilities:

1. Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
2. Coordinates referrals to Hospice to promote continuity of care
3. Provides individualized counseling to patient and family regarding hospice services
4. Establishes and maintains professional working relationship with hospital staff, assisted living facility staff, skilled nursing facility staff, physicians, and VNAHG staff
5. Ensures compliance with regulatory and statutory requirements
6. Adheres to the organizations policy in regards to absenteeism and appearance
7. Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications:

1. Bachelors degree in Nursing preferred
2. Valid NJ Registered Nurse license
3. 2 years off hospice nursing experience
4. Excellent verbal and written communication skills
5. Excellent interpersonal and organizational skills
6. Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
7. Strong decision making skills

Working Conditions/Physical Demand : Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and drivers license.
