Job Summary:

Through a strong Christian worldview, assists residents in addressing spiritual, physical, behavioral, and emotional needs. Provides dedicated care to residents; identifying needs, determining eligibility and referral for a variety of social services, and guiding women through the program helping to provide holistic stability and self-sufficiency.

Supervisory Responsibilities:

• This role does not currently supervise other staff; however, this role will supervise volunteers and interns as applicable.
• Coaches, corrects, rewards, recognizes, and reviews volunteers and interns in accordance with TCM’s Core Values & policies.

Essential Job Functions:

Develops Plans with Residents

• Interviews residents to determine needs and refers them to appropriate community resources.
• Develops resident Individual Service Plans after a needs assessment oral interview.
• Administers assessments to identify needs and track progress of residents and their families.

Assists in Coordinating Care

• Acts as liaison between residents, organizations, and agencies, advocating to address issues facing assigned residents.
• Provides emergency assistance, crisis intervention, and needed referrals.
• Collaborates with Laura’s Home team to guide and support residents.
• Updates resident files by recording each contact made with residents and families, and maintains records required for follow-up.
• Provides a timely response to the needs of all residents on case load.

Other Responsibilities

• Leads scheduled programming to facilitate resident development.
• Actively grows and pursues continued education in community-based services and domain knowledge.
