JOB SUMMARY:

As a member of the Community Health Team, the Substance Use Care Coordinator works with patients and their families/ caregivers to identify their health goals. Together with the primary care provider and patient, the care coordinator will create a care plan that will support the patient in achieving their goals. Responsible for providing care coordination, transitional care interventions, community resource referrals, behavioral health brief intervention and counseling as well as mental health and substance misuse screening and assessment. This position will aim to develop a therapeutic relationship with patients, while assisting them through the process of better health by providing counseling support, encouragement, and education. Must understand substance use, American Society of Addiction Medicine (ASAM) criteria, family systems, community resources, insurance, regulatory and legal system issues relevant to improved health outcomes for patients. This position works as an integral part of the primary care team providing direct service within patient exam rooms and scheduled office visits.
