Human Service Agency is Hiring a Community Services Assistant Coordinator Near Watertown, SD

The mission of New Horizons is to support people to achieve self-reliance and to exercise control over their lives. The Community Services Assistant Coordinator is responsible to guide, counsel, and provide supports for people. Support people to live the life they desire by achieving their goals, dreams, and desires while also providing protections for their health, safety, and welfare.
Position Details:
Full Time: 40 Hours per Week (Monday - Friday). Rotating on-call schedule.
Shift: Days
Pay: $18.50 per hour
Job Knowledge and Comprehension:
A. Understands and demonstrates a commitment to supporting people in achieving their hopes, dreams and wishes (personal outcomes).
B. Serves as a member of the person support team, which designs the personal support plan for each person.
C. Ensure people’s health, safety and welfare is not compromised through unreasonable or unnecessary risk.
D. Monitors needed supports for people supported.
E. Helps people manage their funds.
F. Acts as an advocate for people making sure that people’s rights are not violated and ensuring needed supports are secured.
G. Knowledgeable of assessment and other processes used to discover people’s hopes, dreams, wishes, and preferences. Uses the information that is discovered from the assessments and other processes to help develop a Personal Support Plan.
H. Monitors crisis situations, discusses the incident with authorized staff and people supported. Documents information from pertinent conversations with others. Adjusts supports as recommended and needed to help the person remain safe and healthy.
I. Knowledgeable of and complies with regulations for reporting to the Division and other agencies.
J. Serves as a liaison for people, their families, guardians and other agencies.
K. Provides services, supervision, and accountability of services and activities for people to ensure each is protected from physical, emotional, sexual, or psychological abuse or mistreatment.
L. Ensure that there is a proper amount of staff assigned to shifts as recommended by the Division of Developmental Disabilities and the management staff at the Human Service Agency.
M. Other Duties as assigned consistent with non-exempt functions.
High School Diploma or GED Required.
Two years’ experience working with people with disabilities is preferred.
Supervisory experience is preferred.
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security. Questions about EPHI and HSA Policies should be directed to the HSA Security Officer. Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.
