Human Service Agency is Hiring a Support Coordinator Near Watertown, SD

DESCRIPTION OF POSITION:

The Support Coordinator is responsible for the monitoring of activities and supports pertinent to the supports provided by New Horizons to the participants assigned to their caseload. A key responsibility will be to work closely with all stakeholders in achieving ongoing excellence in supports provided by New Horizons in compliance with all policies and procedures, state and federal regulations, and requirements of CQL accreditation.

Position Details:

Full Time: 40 Hours per Week (Monday - Friday)

Shift: Days

Pay Range: $17 - $18

Role and Responsibilities:

  • Responsible for maintaining and upkeep of people’s files and records.
  • Serves as the primary contact person to the person on assigned caseloads when assistance is required.
  • Participates in all planning meetings, assisting with the Person-Centered Planning, and implementing team recommendations.
  • Delegates the completion of assessments as assigned by the Case Manager.
  • Provides input related to the ICAP assessment to the Case Manager.
  • Reviews completed support plan, ISP monitoring and ensures appropriate follow up with team members.
  • Maintains a positive and professional working relationship with participants, families, guardians, staff, state agencies, case managers and other professionals.
  • Communicates with the case manager to convene the team to review the person’s plan with problems or changes that call for review when indicated.
  • Ensures that new people supported are properly orientated to the agency.
  • Maintains all program documentation necessary as required by agency policy, and applicable accrediting/licensing bodies.
  • Assists people to identify goals/objectives for participants and individualized services and supports needed to achieve desired outcomes.
  • Ensures that each person is receiving assistance in assuming management of those activities for which the person has demonstrated management capacity.
  • Ensure that the participant’s rights and due process are upheld. Ensure when a participant’s rights and/or due process have been restricted that the service team and the appropriate committees have reviewed the restriction, within the required time frame.
  • Completes, obtains, disseminates, updates and files all applicable forms/reports.
  • Co-plans, conducts, and attends extracurricular activities, workshops, in-services, and meetings as necessary at New Horizons or in the community. Travel may be required.
  • Assists in transfers of participants to another service or CSP when the participant desires such transfer, and such transfer is consistent with their service plan.
  • Advocates for participants. Ensures participant choices and preference are elicited and respected.
  • Utilize community resources in the development of programs and services.
  • Ability to assist participants in accessing resources and interacting in the community.
  • In conjunction with other team members, determines specific goals and objectives for participants, and the methods by which these goals and objectives are accomplished and complete in a timely manner.
  • Reviews and compiles participant files selected for the SMART review.
  • Completes all required forms to DSS, housing, and other entities as needed, if we are payee.
  • Reconciles personal and payee accounts monthly.
  • Complete monthly data reports to determine what is working/not working in the action plan, or other areas of the participant’s life that may need addressed.
  • Assist participants with reaching their dreams, hopes, and desires.

Licenses/Required Testing:

  • Valid driver’s license and evidence of insurability.
  • Must be able to successfully complete agency driver’s training course.
  • Must be able to pass a pre-employment drug test.
  • Successful completion of criminal background check.
  • Ability to complete all necessary orientation training (CPR, First Aid, Behavior 101, OSHA, Rights and Abuse, and any other required training)

High School Diploma or GED

Satisfactory work related and character references

Knowledge of computers.

Experience with people with Developmental Disabilities is preferred.

The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security. Questions about EPHI and HSA Policies should be directed to the HSA Security Officer. The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.
