Job Description:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Medication Assistant to join our team in providing exceptional personal care services. The ideal candidate will play a crucial role in ensuring the well-being and comfort of our residents/clients while upholding the highest standards of care.

Responsibilities:

• Deliver personalized care to residents based on individual needs and care plans
• Perform nursing assessments and assist in developing personalized care plans
• Administer medications and treatments as prescribed by healthcare professionals
• Monitor residents/clients' health status and promptly address any changes or concerns
• Collaborate with caregivers, families, and healthcare professionals to ensure comprehensive care
• Provide education and support to residents/clients and their families regarding health conditions and care plans
• Maintain accurate and updated documentation of care provided

Qualifications:

• Valid LPN license in Pennsylvania or Medication Administration Experience Preferred
• Experience in personal care, assisted living, or a similar setting preferred
• Strong knowledge of nursing principles and practices related to personal care
• Compassionate and empathetic approach towards residents/clients
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
