Scope of Work

The ENGAGEMENT SPECIALIST (ES) is an integral member of the Engagement Team. The position provides professional, courteous, and friendly assistance to our customers. The ES serves as a resource for internal and external customers by utilizing resources such as established Gellert Health policies and procedures to ensure delivery of professional care while contributing to the organization’s values of Kindness, Integrity & Quality.

While conforming to state regulations and Gellert Health policies and procedures, the ENGAGEMENT SPECIALIST (ES) has 4 primary areas of responsibility:

1. OUTREACH AND EDUCATION OF POTENTIAL PATIENTS AND COMPLETION OF THE GELLERT HEALTH REFERRAL PROCESS

2. COLLABORATION, EDUCATION, AND DEVELOPMENT OF CURRENT AND POTENTIAL REFERRAL PARTNERS

3. ENGAGEMENT OF PATIENTS AT TIME OF TELEPHONIC NAVIGATION DURING THE YOU DO PHASE AS NEEDED

4. ASSISTANCE IN PROGRAM DEVELOPMENT AND EXPANSION WHEN NECCESSARY

MATERNITY PROGRAM

The primary focus of this role will specialize in identifying a group of high-risk expectant mothers for Healthcare Navigation to outreach and engage with the patients, and connect them with the necessary services and support to achieve improved health outcomes. The Maternity Engagement Specialist will also work collaboratively with the Engagement Team and the Navigation Team to support the Company’s overall goals and priorities.

NECESSARY SKILLS

• Outreach telephonically to ~minimum 50 potential patients a day to connect them with Gellert Health services
• Ability to drive personal vehicle to engage with patients as needed
• Experience with prenatal and post-natal care and high-risk pregnancies (preferred)
• Solid understanding of physical and behavioral health services
• Experience working with culturally diverse, chronically homeless, and
• vulnerable individuals, in medical and behavioral health services (preferred).
• Ability to communicate effectively telephonically
• Excellent written & verbal communication skills
• Comfortable working in a fast paced, sometimes stressful environment with a strong focus on customer service.
• Resourcefulness, close attention to detail, able to work independently and within a team
• Bilingual (preferred) – Spanish highly desirable not required.

REQUIREMENTS:

· At least 21 years old

· 39 months clear Motor Vehicle Record

· Current Arizona driver’s license and Auto Insurance

· An Arizona registered vehicle

· Ability to successfully pass a criminal background check. (Any convictions will be evaluated based upon the conviction in relation to the job requirements. A criminal conviction is not necessarily a deterrent to employment.)

· Ability to successfully pass drug screening

· Associate degree or bachelor’s degree, or work experience equivalent

Pay: $21.00 - $24.00 per hour
