Summit Healthcare Services is Hiring a Transitional Care Navigator Near Phoenix, AZ

Summit Healthcare Services is an innovative healthcare company offering escalated healthcare careers. Summit has a one-of-a-kind progressive continuum of care, offering services for clients in any stage of aging, from Living Well, Home Care and Therapy to Skilled Home Health, Palliative and Hospice.

We're looking to add a Transitional Care Navigator to our team. In this role, this person collaborates with our team, the patient and their caregiver/family and their primary care physician to monitor the patient's condition and make recommendations for services as needed. The Transitional Care Navigator develops an action plan for services for the patient according to their personal goals which could include palliative or end-of life care when eligible or coordinate other medical services as appropriate, transitioning patient between different levels of care at Summit based on their needs. The Transitional Care Navigator acts as a patient advocate in the continuum and is available to the physicians, patients and their family as a resource to facilitate communication.

Responsibilities:

  • Collaborate with patients/caregivers to ensure care is coordinated across continuum involving post-acute transitions to help patients' stabilize when back in their home
  • Facilitate and promote a collaborative process with excellent communication between all health care team members and patients/clients/families
  • Verify insurance and patient demographics, call hospitals and skilled nursing facility to communicate our services and coordinate care, follow up with patients after clinical appointments/events/hospitalizations, and relay changes of condition to the team
  • Facilitate smooth and timely care for high-risk patients in the outpatient setting such as recommending the patient to be referred for end-of-life care when the patients’ condition cannot be improved upon and comfort care is elected
  • When medical professionals determine the patient is appropriate for end-of-life care, this navigator should discuss the Medicare hospice benefit with patient/family, collect records that support a terminal prognosis, gain support from PCP, schedule an RN eval/admission, input records into the EMR, schedule DME/other medical supplies, and facilitate lab/radiology/supply orders from vendors as requested by care team

Requirements:

  • Medical Assistant that has knowledge of chronic illnesses w/ physical and pyscho-social effects, who knows how to read charts/labs/radiology reports with clinical understanding of diseases, symptomatology, and diagnostics (required)
  • This person should be assertive and creative in problem solving, skilled in negotiation and conflict resolution (required)
  • Knowledge of, and ability to use, electronic record keeping systems (EMR, insurance carrier portals, spreadsheets, hospital systems, Practice Fusion, EClinical Works and Athena EMR preferred)

Job Type: Full-time

Pay: $22.00 - $25.00 per hour
