1. Engage patients and facilitate psychoeducation and life skills groups

2. Support Unit programming by adhering to hospital schedule

3. Engage in CFT's and all paraprofessional aspects of case management

4. Appropriately document all patient contact in the EMR

5. Maintain contact with patient support system as directed by therapist or social services manager

6. Attend clinical oversight

WORK ENVIRONMENT:

This job operates in a clerical office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

PHYSICAL DEMANDS:

This is largely a sedentary role; however, walking, standing, lifting, bending and stooping is required.

TRAVEL:

No travel required.

REQUIRED EDUCATION / EXPERIENCE:

• Bachelor’s degree in SW or related field.
• Organizational skills
• Ability to work in a fast-paced environment
• Adherence to Destiny Springs policy and procedures
• Collaborate with all disciplines and team members
• Exceptional interpersonal, verbal communication, active listening and critical thinking skills
• Fingerprint clearance card or ability to obtain
• CPR certification, Basic Life Support, Behavior Management Training required prior to patient interaction.

Pay: Up to $26.00 per hour
