The Chaplain provides spiritual counseling or arranges for counseling to meet the spiritual needs of patients and families in a manner consistent with their beliefs and desires and in accordance with the patient's plan of care. Provides direct spiritual counsel to patients/families as requested.
Schedule: Part-time 24 hours per week, 3 days a week, 8 hour days - generally from a 9a-5p. One weekend every other month, on-call 1 weekend every other month, and 1 holiday per year. Days off can be set each week or rotate. Thursdays and Fridays are required working days each week.
Essential Job Duties:
- Completes initial spiritual assessment
- Contacts patient's clergy as requested by patients and family
- Establishes visit plan and spiritual plan of care
- Provides prayer and sacraments to patients/families as requested
- Is alert to the theological/ethical issues which may be of concern to patients/families and team members
- Assist in planning funeral services, prior to death, as requested by patient's family
- Works with volunteer services department to place spiritual care volunteers with appropriate patients and families
- Serves as liaison with local religious community
- Provides support and counsel to clergy working with patients/families admitted to Unity's programs
- Provides an awareness of hospice and palliative services and death education to the religious community
- Maintains contact with the continuing education resources for clergy and religious caregivers in the area
- Collaborates with the bereavement team in the planning and offering of memorial services and programs
- Attends IDT meetings
- Participates in the development of the initial plan of care, and provides input into the review and revision
- Serves as a resource to other patient-care staff on patients/families spiritual belief systems

Requirements
Qualifications
- Bachelor's degree from an accredited school of theology, religious studies, or related studies and/or certification in hospital or pastoral ministry required. Master's degree in theology, divinity, or pastoral studies preferred
- Two units of Clinical Pastoral Education preferred
- 2 years of Chaplain experience required
- Experience in hospice and palliative care and an understanding of hospice and palliative philosophy is preferred
- Valid Wisconsin Driver's License, reliable transportation, and proof of current automobile insurance coverage
- Internet access to accommodate charting and laptop synchronizing capabilities
