The Client Care Coordinator works under the supervision of the Administrator of County Services and the general direction of the Deputy Director. The Client Care Coordinator performs professional assessments of an individual’s capabilities and the family’s capacity to provide care. They are responsible for the client referrals, resource information , multi-program assessor and coordinator.

Requirements

• Experience in Care Management or
• Administrative Experience in working with elderly and/or disabled community or
• Social Services type Degree or
• BA or BS in Related Field or
• LPN Degree Eligible

• 8 hour shift
• Monday to Friday
