Statement of Purpose In accordance with directives from the Cardio Services Practice Director and Medical Providers, the office nurse coordinates and administers nursing care for medical patients who present to the practice for care. Participates in providing informational education to patients or caregivers. Contributes to the assessment of the health status of the individuals. Participates in the development and modification of the strategy of care for individuals. Participates in the evaluation of responses to interventions. Maintains safe and effective nursing care rendered directly in indirectly. Works cooperatively with other patient team personnel in maintaining standards for professional nursing practice in the physician office setting. Maintains a cooperative working relationship with other team personnel and maintains standards of behavior of NC Cardiology while striving to meet the goals of the clinic and North Country Health Systems as a whole. Essential Functions, Duties and Responsibilities Reports to Director of the Practice. Receives and greets patients in a professional and friendly manner. Ascertains patient#s needs, obtains history of current complaint and details of past medical history. Responsible for checking vital signs, performs EKGs, reviews allergies, medical history, and performs medication reconciliation. Is responsible for recording all pertinent medical information into the medical chart according to established rules and requirements. Draws labs for patients, when needed. Explains normal and abnormal test/lab results to patients and their significant others. Educates patient regarding medications. Works in conjunction with Case Management, Chronic Care Coordinators to facilitate comprehensive cardiology care for patients. Schedules and processes paperwork for patient tests, cardiac catheterizations and other invasive procedures, labs, device procedures or office appointments. Calls in prescriptions. Arranges hospital admissions. Provides patient education on invasive and non-invasive procedures and diagnostic tests. Triages appointment referrals to ensure patients are being seen in a timely manner. Triages phone#calls and clinical questions. Orders prescriptions and manages refills as directed by healthcare provider. Orders tests, labs and referrals as directed by the healthcare provider. Reviews charts prior to the patient#s appointment, making sure all referrals, lab results, etc. are in the chart prior to the physician seeing the patient. Schedules patients for referrals and sends necessary papers to accompany the patient. Keeps track of all appointments set up outside of the practice. Makes sure reports and documentation of that appointment are received by the referring provider and becomes a permanent part of the patient#s chart. This would include but is not limited to CT/MRI screening forms, lab results, etc. Appointment confirmations are made by telephone, face to face or by sending a letter with the information to the patient. Works closely with the Stress Lab Staff and assists with Stress Testing procedures as needed (Nuclear Exercise Stress Tests, Lexiscan, Dobutamine, Cardiac, Echo Stress, etc.). Maintains the standards of nursing care and implements the policies and procedures of the hospital. Make sure any outdated medications are returned to the pharmacy for disposal and orders replacements. Courteously speaks with patients or healthcare providers who call the office with medical concerns, assessing patients with physical complaints and making appropriate recommendations based on that assessment, including recommending the patient be seen in the office or in the emergency room. Follows up on telephone contacts as needed. Makes providers aware of all such decisions through discussion and/or progress notes in the medical record. Triage and prioritize medical/emergency phone calls; handles according to office protocol. Provides for privacy # modesty needs of patients. Uses age appropriate and familiar terminology. Encourages participation in planning of care and provides for emotional needs. Communicates face to face with patient. Allows time for patient to respond to communication. Is responsible for daily preparation and upkeep of examination rooms. Assists and completes various forms for the patient. Maintains inventory of medical supplies and orders new supplies as needed. Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner. Reports broken or malfunctioning equipment to the Practice Director. Maintains a safe environment for patients and staff according to VOSHA guidelines. Attends in-service training programs as directed. Participates in establishment and maintenance of policies and procedures relating to clinical activities including quality assurance and compliance with OSHA regulations. Establishes and maintains communications and utilizes teaching opportunities with patients and other significant individuals including education regarding diet, medications and testing needs. Assists with or institutes emergency measures for sudden adverse developments in patients such as medication reaction, severe bleeding, and triages telephone calls from patients with a medical problem. Assists with the development of clinical processes, procedures and workflows with the Practice Director and Medical Provider. Maintains patient confidentiality and follows department policies regarding release of information. Coordinates nursing care of patients when scheduled for procedures by other departments. Backs up other office personnel as needed. Performs other duties as directed by the Practice Director. Understands and complies with the North Country Hospital Standards of Behavior and models ethical behavior as expected by our patients, colleagues and the community.
