Description JOB SUMMARY The Stroke coordinator is responsible for coordinating and implementing services and activities related to the Stroke Center. He/she functions as a liaison between the physicians, nursing staff, allied healthcare professionals, patients and their families. Coordinates and facilitates the care for the stroke patient throughout the continuum. Works closely with physicians and nursing to ensure clinical care path is followed, as appropriate. The stroke coordinator participates in program development as it relates to the stroke center. This includes, but is not limited to staff education, community education, and marketing. Responsible, in coordination with the Director, for planning, developing and implementing stroke center initiatives. Will maintain quality standards for responsive service including data management of key quality indicators. He/she will take a leadership role in development and implementation of outcomes evaluation as guided by regulatory and accreditation standards. GENERAL REQUIREMENTS Adheres to the hospital and departmental attendance and punctuality guidelines Performs all job responsibilities in alignment with the core values, mission and vision of the organization Performs other duties as required and completes all job functions as per departmental policies and procedures Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs) Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time. Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills. Must possess and maintain a valid Georgia Driver's License and meet auto insurability requirements as approved by Phoebe Putney Health System's insurance company. WORKING CONDITIONS General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels. May be exposed to limited hazardous substances or body fluids, or infectious organisms. May be required to change from one task to another of different nature without loss of efficiency or composure. Periods of high stress and fluctuating workloads may occur. Long-distance or air travel as needed - not to exceed 10% travel.
