Position Summary: The Health Information Management (HIM) Coordinator functions within the Integrated Health Care division of Liberty Resources. Integrated Health Care service portfolio includes behavioral health, primary care and substance use. The HIM Coordinator will manage various medical record functions, including distribution and tracking of admissions, discharges, and transfers (ADTs) from internal programs and external acute care settings. The HIM Coordinator serves as an essential link between patients and their care team.Job Responsibilities: Intercepts and coordinates all ADT notifications from regional health information exchanges (HIEs) to ensure information is appropriately communicated with patient care teams in a timely manner.Collects and enters medical records from acute care settings into the electronic health record, i.e. discharge summaries, H&Ps, L&D reports, diagnostic reports, etc.Tracks all ADT notifications to ensure the care team has completed appropriate follow-ups as indicated by the discharging facility.Ensures Integrated Health Care Clinic census is accurate by reviewing and updating medical records, as applicable, for all internal admissions, discharges, and transfers.Gathers and generates reports on various department metrics for review by management; recommends processes and procedures to improve departmental performance.Performs other responsibilities as assigned.
