We are a 120-bed not-for-profit nursing facility offering advanced healthcare while providing skilled nursing and rehabilitative services for both long and short term residents. Located on eight beautiful landscaped acres, we also provide sub-acute services, wound care and respite services.

QUALIFICATIONS:

Must be a graduate of an APTA accredited Physical Therapy program with a Bachelors or Masters degree in Physical Therapy. Must be currently licensed and registered to practice Physical Therapy in the State of New York. Must have 3-5 years experience as a Director, Assistant Director or Supervisor of Physical Therapy in a long-term care setting.

RESPONSIBILITIES INCLUDE, BUT NOT LIMITED TO:

Responsible for the management of physical therapy services, the provisions of physical therapy evaluations, interventions and the development and implementation of programs and protocols. Takes responsibility for completion of rehab sections on MDSs. Participates in QAPI, PDPM projects and in rehab review meetings. Provides supervision for Rehabilitation staff and volunteers.

Hours and days of the week may vary depending on employment status and facility needs. NON - EXEMPT
