Orlando Family Medical is looking for an experienced Case Manager who accomplishes client's care by assessing treatment needs; developing, monitoring and evaluating treatment plans.

Requirements:

Must have knowledge of coordination of care

HEDIS

Great written and oral communication skills

Know medications

At least 5 years of experience

Must be a Registered Nurse

Must be bilingual (English and Spanish)

Full Time Position, hours are Monday through Friday 8:00am-5:00pm

Pay: $15.00 - $18.00 per hour
