About the Organization and the Job :
Heart of Hope Asian American Hospice Care is a non-profit startup Hospice licensed by California state since 2016. We are looking for a per-diem Home Health Aide (HHA) to join our growing team in San Jose, California.
It offers a unique opportunity to work with a team that is passionate in providing end-of-life care to the under-served community in the Silicon Valley Area.
High-level Job Description
Under the supervision of the registered nurse, the HHA provides personal care and related non-professional household services necessary to meet the hygiene, ADL, and comfort needs of the homebound patient in alignment.
The HHA receives functional guidance and supervision from the interdisciplinary team. The HHA will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
Pay Rate and Benefits
Pay rate depends on relevant qualification and experience.
Hourly rate : $22-$27
Visit rate : $50-$55
