Overview: 
The Community Support Specialist (CSS) plays a crucial role in supporting individuals reentering the community from incarceration by breaking down the common barrier of transportation. The CSS supports participants’ ability to attend medical, dental, and psychiatric visits; obtain identification forms such as birth certificates, social security cards, and drivers licenses; and also ensures that participants can access other important resources such as food stamps and Medicaid insurance. In addition to transporting participants, the CSS has the opportunity to impact the participants through regular conversations and rapport-building.

Your role will involve:

  • Providing transportation and support to participants:
    • Building rapport with participants using a trauma-informed approach with high-risk individuals
    • Reviewing daily schedule to coordinate participant transportation needs in the community
    • Communicating with the Center Coordinator on any schedule changes throughout the day
    • Transporting participants to scheduled appointments, AA/NA meetings, and part-time employment
    • Ensuring participants have necessary documentation and funding for appointments
  • Collaborating with Concordance team members:
    • Documenting all transports within Concordance’s electronic record system directly following appointments
    • Collaborating with team members, supporting a positive, mission-focused work environment
    • Maintaining good working condition in Concordance-owned vehicles (scheduling vehicle maintenance, coordinating needs for rental vehicles, etc.)

Skills, Knowledge, and Expertise

The right candidate will come with: 
  • Passion for our mission
  • Strong communication and interpersonal skills 
  • Ability to build appropriate, supportive, trauma-informed relationships with participants 
Required education, licensure, and experience: 
  • Valid, Class E Driver’s License and comfortability transporting groups in a vehicle provided by Concordance
  • High school degree of equivalent
  • 2 years of experience in social service or transportation field preferred
  • CPR / Adult First Aid certification preferred
