Job Description:
Our client is seeking a Registered Nurse to join their mission-driven team in Los Angeles, California. The ideal candidate will be passionate about delivering high-quality, integrated care to frail older adults and will play a critical role in the management and delivery of direct nursing care to PACE participants. Our client offers a robust compensation package for this role that includes cash compensation and other total rewards. Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role.
Key Responsibilities
Deliver direct nursing care to PACE participants in various settings including clinic, care center, home, virtually, and SNF
Conduct nursing assessments, inclusive of physical, psychosocial, and behavioral status
Educate participants, caregivers, and staff on how to personalize and carry out care plans and respond to situations that arise in each participants life
Monitor and evaluate therapeutic interventions, and recommend changes as needed to proactively improve health and wellbeing
Schedule and Shift Details

• 40 hours per week, Monday to Friday, with occasional on-call coverage

Requirements:

• Associate degree or higher
• 1 years of experience in ALL of the following:
• Burn Unit/Wound Clinic
• Wound Care
• Phlebotomy
• 2 years of experience in ANY of the following:
• Geriatric Care
• Senior Care
• Geriatric/LTC
• Valid Alzheimers certification
• Valid American Heart Association Basic Life Support (AHA BLS)
• Valid Registered Nurse (RN) in the state of California (CA
