Cape Cod Healthcare is Hiring a Pastoral Counselor Near Hyannis, MA
- Demonstrates an understanding of hospice philosophy while providing direct non-denominational spiritual support to patients/families in keeping with patient/families beliefs.
- Work with staff, clergy, and community groups to enhance their sensitivity to the pastoral/spiritual concerns of patients/families experiencing terminal illness and loss.
- Assist with and provide pastoral/spiritual support to members of the Interdisciplinary Team.
- Participate in patient conference by exploring and assessing the potential pastoral/spiritual needs of patient’s families and reporting on services as indicated.
- Provide pastoral/spiritual follow-up services for bereaved addressed by hospice team.
- Document assessments and re-visits in an accurate and timely manner in accordance with CMS regulations.
- Make contact with clergy or appropriate representatives of patients/families as indicated.
- Perform occasional liturgical service as requested by patient/family.
- With the Bereavement Coordinator, plan and conduct memorial/remembrance services with staff.
- Makes arrangements for funeral or memorial service when indicated.
- Attend patient deaths as requested by patient and/or family.
- Develop and maintain a resource group of clergy to whom specific aspects of pastoral/spiritual care may be delegated.
- Participate in on-call services as needed and required by the needs of the program.
- Provide educational programs for community clergy, religious and lay representatives as resources allow.
- Assist in educational training program for Hospice/Palliative Care staff and volunteers.
- Other duties as assigned.
- Degree from accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry. Master’s degree preferred.
- Demonstrated ability to be open, sensitive, flexible and interreligious.
- Experience in working with patients/families dealing with life-threatening illness and death.
- 1 or more units of CPE preferred.
Schedule Details37.5 hours per week, Mon-Fri, 8a-4p, Occ Weekends & No Holidays
