Registered Nurse (RN)LOCATION: East Brunswick and Central New Jersey
Position Description
- Plan and provide clinical support to In-Home/Residential patients
- Provide and coordinate patient care, educate patients
- New client intake
- Educate and train staff on all required medical trainings
- Document client care services by charting in client records
- Administer and audit medications and medication administration records
- Perform wellness checks as needed
- Coordinate with client's interdisciplinary team, including maintaining a cooperative relationship among healthcare teams by communicating information; responding to requests and building rapport
- Ability to be flexible and adjust to the needs of the program
- Attend interdisciplinary team (IDT) meetings
- Experience working with individuals with developmental disabilities preferred
- Position Requirements:* Valid New Jersey Registered Nurse License Valid New Jersey Driver's LicensePush, pull, and lift up to 50lbs
