Liberation Programs, Inc. is Hiring an Admissions Coordinator Near Bridgeport, CT

Liberation Programs is hiring a Full Time Admissions Coordinator to perform In-take Assessments for individuals entering our Residential Treatment Programs for Substance Use Disorders. The Admissions Coordinator will assist in assuring expedient admissions of persons seeking treatment, conducting assessments, assigning the person to the appropriate level of care and ensuring that programs are at capacity.

DUTIES

  • Coordinate and manage agency admission process
  • Schedule intakes
  • Maintain schedule
  • Place reminder calls to clients about their appointment
  • Place follow-up calls to clients who miss their appointment
  • Complete assessments (Intake paperwork - complete and accurate)
  • Conduct HIV risk assessment and inform client of the Ora-Sure test
  • Complete psychiatric MINI and Medical History Form
  • Complete financial authorizations for initial assessment and refer client to the Entitlement Specialist for follow-up sessions
  • Refer and schedule an appointment with the entitlement specialist for all clients who are unemployed, without benefits, and might be eligible for benefits
  • Recommend clinically appropriate referrals while satisfying the needs of referral sources
  • Coordinate with the Team Leader for client assignment to primary clinicians
  • Follow-up with referral sources to communicate client status, including documentation and or telephone communication
  • Ensure clients are admitted to treatment within 72 hours of initial contact with agency, either by telephone or walk in
  • Report weekly on new admissions using the AWARDS admission reports
  • Provide Medical Director/ Physician with all required information to complete medical assessment
  • Ensure utilization remain above 90%
  • Conduct outreach at the request of the Director
  • Collaborate with other agency departments to increase interagency referrals and use of available treatment slots

QUALIFICATIONS

  • Bachelors degree and/or CAC
  • 2 years experience working with similar population
  • Experience in evaluating and determining appropriate service needs for individuals needing care
  • Experience with collecting data and generating reports
  • Excellent writing and verbal skills
