With a census of 20-25 patients, we specialize in pediatrics, geriatrics, ODP waivers, and handling behavioral and mental health conditions. We adhere to CMS guidelines and CHAP certifications to ensure the highest standards of care.

Position Overview:

We are seeking a dedicated and experienced Director of Nursing (DON) to join our dynamic team. The ideal candidate will be a team player committed to helping grow our census and support our team. A background in pediatrics is preferred, along with home health and current OASIS experience, although hospital experience will also be considered. The DON will handle compliance, documentation, supervisory visits, and training of caregivers.

Key Responsibilities:

• Oversee the clinical operations of the home health agency, ensuring compliance with CMS guidelines and CHAP certifications.
• Manage and support a census of 20-25 patients, predominantly pediatric cases with various behavioral and mental health conditions.
• Conduct supervisory visits to caregivers, ensuring high-quality care and adherence to care plans.
• Provide training and development for nursing staff to ensure best practices in patient care.
• Handle compliance and documentation, ensuring all records are accurate and up to date.
• Collaborate with the administrative team to promote growth and improve patient outcomes.
• Participate in on-call duties as required, providing support and guidance to staff as needed.
• Travel within the surrounding area for supervisory visits and patient assessments, with mileage reimbursement provided.

Qualifications:

• Licensed Registered Nurse (RN) in good standing in the state of Pennsylvania.
• Minimum of [4] years of nursing experience, with a preference for pediatric and home health care experience.
• Experience in a management position within sub-acute care.
• Current OASIS experience preferred; hospital experience can be considered.
• Strong knowledge of CMS guidelines and CHAP certifications.
• Excellent communication, organizational, and leadership skills.
• Ability to manage multiple tasks and priorities effectively.
• Willingness to be on the road within the surrounding area.
