Job Title: Licensed Practical Nurse (LPN)
Location: State Long-term Care Facility, Phoenix, Arizona
Job Type: Full-time contract; 2-year contract
Pay: $44/hr
6:00-6:30; Days & Nights
TEEMA Solutions is making a meaningful impact on the lives of geriatric residents. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care and support in a professional and prescribed manner working under contract at a state of Arizona facility.
Responsibilities:
As an LPN with us, you will:
Administer medications and perform various treatments as ordered by physicians within your scope of license.
Observe and report the effectiveness of medications and treatments.
Ensure adequate supplies and medications are available for resident care.
Complete initial admission nursing assessments for new residents.
Assist in the development of Minimum Data Set (MDS) assessments and care plans.
Maintain accurate clinical and administrative documentation.
Supervise and assist subordinate staff with resident care, including Activities of Daily Living (ADLs).
Actively participate in the Arizona Management System (AMS) for problem-solving and work process improvements.
Contribute to our Quality Assurance and Performance Improvement (QAPI) program.
Conduct in-service training and staff development programs.
Attend and engage in staff meetings.
Qualifications:
Valid LPN license issued by the state of Arizona.
Mimimum 2 years long-term care or skilled nursing experience
BLS issued by the AHA
Current AZ Fingerprint Card
All state requirements to practice as an LPN in AZ
Excellent communication and interpersonal skills.
Ability to work effectively with interdisciplinary teams.
Proficiency in clinical documentation.
Commitment to continuous improvement and professional development.
