Major Tasks, Key Responsibilities and Key Accountabilities
— 1 Logs each referral received in the Smartsheet.
— 2 Request initial Authorization dependent on Insurance and Agency policy
— 3 Accurately fills out intake information into active admissions on every Start of Care (SOC)
— 4 Accurately and completely enter the patient information into the EMR for all SOCs per agency process
— 5 Schedules the SOC OASIS and other tasks to the appropriate scheduler
— 6 Scans and attaches all clinical support documentation to the MISC tab and verifies eligibility check
— 7 Provide support to other members of the department as needed
Flexibility to work an on-call weekend/holiday if needed.
