FAMILY SERVICE LEAGUE is Hiring a BILINGUAL CASE MANAGER Near West Hempstead, NY

SUMMARY

Family Service League is seeking a full-time Bilingual Case Manager for the Healthy Families Nassau program. The Bilingual Case Manager will be responsible for providing home-based prevention services focusing on child development, education, and coordinator of services/activities for overburdened families of newborns participating in the program. The Bilingual Case Manager will provide home visits to families located throughout Nassau County, but primarily in Hempstead, Uniondale, and Roosevelt. Travel is required.

RESPONSIBILITIES

  • The Case Manager will provide comprehensive in-home services to a caseload of pregnant and new parents in accordance with the Healthy Families program model.
  • Establish a trusting relationship with families by initiating and maintaining regular contact with participants.
  • Partner with families to plan goals and activities to promote family/child health and development, positive parent-child attachment, and bonding, and enhance family functioning.
  • The Case Manager will support participants’ needs by providing referrals, advocacy, follow-up, and overall service coordination with other providers.
  • Assist in making and attending health and human service appointments, including activities related to employment and educational goals.
  • The Case Manager will be responsible for conducting, monitoring, and tracking data related to assessment tools on data system.
  • Conduct community outreach and complete assessments for referred families as needed.
  • All other duties as assigned.

QUALIFICATIONS

Associates degree in social work or related behavioral health field is required. Bachelor’s degree is preferred.

A minimum of 2 years of prior related experience is required.

Demonstrated experience establishing trusting, honest relationships with individuals and family members are required. Prior knowledge of infant and child development, infant mental health endorsement Level I or II is preferred.

Proficient computer skills, including Microsoft office, required.

Experience with culturally diverse populations preferred.

Excellent verbal and written communication skills required.

Strong judgement, emotional intelligence, patience, and empathy, as well as the ability to handle stressful situations required.

Ability to work on a family support team is required.

Valid and clean NYS Driver’s License required.

Bilingual in Spanish or Creole, required.
