Position Title: Residential Services Coordinator Department: ProgramGrade Range/Job Status: Full Time/Exempt-Salaried Reporting Relationship: Chief Program OfficerSupervisory Responsibilities: NA Typical Schedule: Primarily M-F Days. Must occasionally be willing and able to work flexible hours/days, including evenings and weekends, reflective of the dynamic schedule of the organization. Position SummaryThe Residential Services Coordinator will serve as the primary contact for referrals and for interested parties who wish to learn about Hammer & NER residential services. This includes being in communication and relationship with county and contracted case managers. The position will serve as a key contact for Hammer & NER collaborations with the Altair ACO. The position will facilitate the Program responsibilities related to submission of service records to the county, DHS and state agencies. All employees are expected to center, model and champion Hammer’s core values: Person-Centered, Relational, Opportunistic and Stewardship in order to provide people with intellectual and other disabilities, the opportunity to live life to its fullest.Primary Duties and Responsibilities Residential Services Coordination Serves as the primary initial contact for new individuals, guardians, and case managers seeking Hammer & NER services.Provides information to interested parties about Hammer & NER services.Facilitate new family meetings and site tours.Maintain demographic information on referrals and when placement is accepted.Manage the intake process until placement is accepted. Altair ACO CoordinationProvide the coordination of partnership projects related to health information and care coordination, including LifePlan and other potential assessment tools.Facilitate processes involved with DHS incentive grants.Collaborate with internal and external resources to implement Altair ACO initiatives. Funding Systems DocumentationMaintain staffing data and forms related to Individual Service Agreements.Coordinate with Hammer & NER finance department. the completion of required forms and communication related to Individual Service Agreements.Participate in submission of exception requests as needed related to individual funding.Manage communication with government entities to effectively complete responsibilities related to individual funding. Organizational and State Information Systems Use internal data systems proficiently and effectively to maintain information related to the residential services model.Using state funding policies as a guideline, to complete forms and procedures to bill for services properly.Use governmental data information systems related to state funding structure and regulations. Essential Knowledge and Qualifications Intermediate experience required working for a mid-sized non-profit organization, working alongside and on behalf of people with disabilities or applicable work. Formal, informal, and cross-disciplinary experiences will be considered.Experience and ease communicating with a variety of key stakeholders including guardians and families, people with disabilities, case managers and others who may be a part of a person’s interdisciplinary team.Comprehensive knowledge of federal and state licensing regulations and funding mechanisms that relate to Hammer Residences’ services preferred.Ability to work in collaboration with other program leaders, internal departments, as well as external stakeholders effectively.A willingness to manage multiple priorities to ensure work is completed in a timely and productive manner. Demonstrated experience communicating effectively and persuasively in oral and written communication on an interpersonal or group level. Proficiency with Microsoft Office Products (Word, Excel, Outlook). Physical demands include frequent use of a telephone, frequent use of repetitive motion activities including typing on a computer keyboard and the ability to work for several hours at a time at a desk and computer. Must be mobile physically or through the use of an adaptive device in order to provide tours of residential sites.Requires a valid driver’s license, access to a personal auto and willingness to drive as needed to our east and west metro offices, residences and other locales as needed.Experience, ability, and commitment to working with individuals and teams that are mixed across lines of difference such as race, gender-identity, sexual orientation, religion, ability, age, class, and immigrant status. Must be willing and able to work flexible hours/days, including occasional evenings and weekends, reflective of the dynamic schedule of the organization. This job description assigns essential functions.
