```Duties```
- Provide case management services to individuals and families in need
- Conduct assessments and develop individualized service plans
- Coordinate and monitor client progress towards goals
- Advocate for clients and connect them with community resources
- Collaborate with other professionals to ensure comprehensive care
- Maintain accurate and up-to-date case records

This position requires a Bachelor's degree in social work or a related field. Previous experience in case management or a related role is preferred. Strong communication, organizational, and problem-solving skills are essential. The ability to work independently as well as part of a team is important.
