The lead case manager is a care management professional who supports the system director and/or manager of Care Management. Responsible for daily oversight and tasks of the care managers and care coordinators to meet campus throughput and quality goals related to care progression. Develops and implements process and education to facilitate daily work of the team. Actively involved in future planning and development of care management system needs, to include policy development, program development and quality improvement. Collaborates with campus leadership to identify educational and/or process gaps and develop specific plans to address. Responsible for new hire training and education and access to appropriate tools for success. Exhibits critical thinking skills and creativity and manages resources to achieve quality performance, positive employee relations and compliance with hospital guidelines. Trains, mentors and facilitates workflow processes within the care management team in the delivery of services to our patient population. Ensures effective stewardship of scarce resources. Supports change activities and works to ensure implementation of changes that may occur. Builds relationships with colleagues and peers, demonstrating the UF Health Difference standards in all interactions.
