Summary Description: The Care Manager is responsible for delivering comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Westchester County. This includes assessing, overseeing, and coordinating various needs such as medical, behavioral health, substance abuse, and social services. The role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices.
Key Responsibilities
  • Provide comprehensive care management and coordination.
  • Participate in meetings with community agencies and service providers.
  • Follow established agency policies and maintain professional standards.
  • Prepare required reports and documentation.
  • Provide after-hours emergency coverage and crisis intervention as needed.
  • Demonstrate flexibility with working hours.
  • Conduct face-to-face and telephonic care management services.
  • Adhere to standards established by regulatory authorities.
  • Perform other duties as assigned.

  • Knowledge of behavioral diagnoses and symptomatology.
  • Familiarity with community resources.
  • Effective client engagement skills.
  • Understanding of chronic disease/behavioral health interactions.
  • Ability to work independently and efficiently.
  • Proficiency in using Electronic Health Record systems.
  • Practice in a recovery-oriented, person-centered methodology.
Key Performance Indicators (KPIs)
  • Timely and accurate completion of required documentation.
  • Effective coordination and utilization of community resources.
  • Responsive and supportive crisis intervention.
  • Adherence to regulatory standards and agency policies.
Requirements
  • Bachelor's degree in a relevant field or equivalent.
  • E.g. social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field
  • Two years of relevant experience or equivalent. A master’s degree in one of the listed education fields may be substituted for one year of Experience.
  • Basic computer knowledge.
  • Valid Driver's License and satisfactory driving record.
  • Maintain New York State Defensive Driving certification within 30 days of hire.
  • Must maintain pre-employment clearances by applicable regulatory clearances as required.
  • Legal eligibility to work in the United States.
Special Considerations : The role requires flexibility in working hours and availability for after-hours emergency coverage.
