Hours: Full-time | Days 

Territory: This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the service area of where our patients live is in the 11 County Twin Cities Metro Area. The general territory for this position would be the Northeast Metro (the cities that are North of Highway 94 and East of Highway 65).

A few must-haves for this position:

• Graduate of an accredited school of nursing and have a current license as a Registered Nurse in the State of MN.
• Able to travel within the entire geographic area serviced by the office. Reliable transportation, valid driver's license, and proof of auto insurance required.
• CPR Certification
• Two (2) years of RN experience.
• One (1) year case management experience preferred.
• Community health and a wide range of Medical-Surgical experience preferred.

Some functions of what you would do as an RN Case Manager:

• Provides services that are ordered by the physician as indicated in the plan of care.
• Completes nursing initial and ongoing assessments of all in home clients within specified deadlines.
• Participates in ongoing interdisciplinary assessment of the patient.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Visits in home clients to evaluate the quality of care being provided and review clinical notes.
