Shore Community Services, Inc., is looking for a Nurse to provide services in a day program setting. RN or LPN license and intermediate computer skills required.
General Description
A. Oversees information regarding medical needs of individuals served at the Lois Lloyd Center.
B. Serves as resource regarding medical/health issues.
C. Implements general responsibilities.
Qualifications – Education or Equivalent
Practical Nurse or Registered Nurse (preferred) with valid, current Illinois Professional license.
Qualifications – Experience or Equivalent
One year experience with persons with intellectual and other developmental disabilities or experience in community nursing.
Qualifications – Abilities
Collects data, recalls facts, develops, analyzes, evaluates and implements plans.
Qualifications – Skills
English reading/verbal/writing skills, interpersonal, math, computer literacy, organizational, driving, ability to work independently, as well as part of an interdisciplinary team.

Uses hands, sits, bends, lifts, pushes, pulls, carries, independent mobility.

Essential Duties
A. Oversees information regarding medical needs of persons served at the Lois Lloyd Center.
1. Administer medication and tube feeding for person served.
2. Record administration of medication to persons served.
3. Participated in feeding of persons served.
4. Order supplies and maintains inventory of medical supplies.
5. Monitors persons served absences as related to medical issues.
6. Maintains and updates medical record for person served.
7. Participates in Personal Plan meetings or supplies data relative to medical needs.
8. Refers persons served families to multiple medical facilities, doctors and dentists, when
requested.
9. Provides consultation to families/caregivers regarding health issues.
10. Secures appropriate resource materials to be used in consultation.
11. Works with staff to develop medically sound toileting procedures.
12. Conducts and/or assists in persons served training sessions on relevant topics regarding
health and personal skills.
B. Serves as resource regarding medical/health issues.
1. Provides first aid treatment to persons served or staff and others as needed.
2. Assists in developing and implementing health and medical procedures.
3. Arranges for and participates in various health screening programs.
4. Monitors compliance of program with outside agency health/medical regulations and
standards.
5. Alerts Director or Program Managers to problematic health related issues.
6. Conducts In-Services for staff relative to health or medical issues.
7. Makes recommendations to Director relative to health or medical issues.
8. Completes assessments or reports as requested.
C. Implements general responsibilities.
1. Complies with all Organization policies, procedures, rules and guidelines.
2. Maintains confidentiality of persons served and advocates for rights of persons served.
3. Emphasizes safety rules and procedures including Universal Precautions.
4. Recommends risk prevention strategies.
5. Reports suspected or known misconduct, including fraud, abuse or waste by staff or
volunteers in accordance with Corporate Compliance requirements.
6. Reports suspected abuse, neglect or financial exploitation of persons served in required
timelines.
7. Participates in training to upgrade skills.
8. Maintains organized, clean, functional work space and equipment.
9. Serves on internal/external committees or represents Division or Organization at meetings as
assigned.
10. Substitutes for other staff as assigned.
11. Provides supervision to volunteers and interns as assigned.
12. Performs other duties as assigned.
