Job Summary:
We are seeking a compassionate and experienced Registered Nurse to join our Home Health team. The ideal candidate will provide quality care to patients in the comfort of their homes, ensuring their well-being and promoting independence.
Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and symptoms, and report any changes to the healthcare team
- Educate patients and their families on proper care techniques and disease management
- Coordinate with other healthcare professionals to provide comprehensive care
- Document patient progress and medical information accurately
Experience:
- Prior experience in home health or a related field is preferred
- Proficiency in CT, Mammography, ventilator care, Triage, HIPAA regulations, and medical documentation
- Knowledge of Memory care, Dementia care, Spinal tap procedures, and Case management is a plus
