Job Overview:

Performs professional work involving intake services including the determination or redetermination of the eligibility of individuals for human services. Coordinates with the other Agency programs as needed. Does related work as required.

Supervision:

Performs work under the direct supervision of the Intake Manager.

Duties/Responsibilities:

• Interviews individuals to determine their eligibility, securing and verifying financial, social, functional and environmental data by telephone or in person.
• Responds to general information requests via telephone, e-mail or in person and releases information orally or in writing.
• Determines appropriate action steps or referrals according to established procedures within time constraints. Makes referrals to appropriate service and program agencies.
• Explains available programs and rights and responsibilities of recipients.
• Participates in staff development programs and interdisciplinary team meetings. Attends meetings and training sessions.
• Prepares and maintains records, files, and reports. Utilizes automated client tracking and computerized database system.
• Assigns cases to Care Coordinators according to established procedures. Acts as liaison for clients to facilitate services.
• Collects and maintains community service/program data. Utilizes computerized resource database system.
• Performs other related duties as assigned.

Knowledge, Skills, and Abilities:

Knowledge of the principles and practices of public social/human service organizations and public assistance programs. Knowledge of current social, economic, and health problems and of human behavior and social functioning. Knowledge of the techniques and means of interviewing. Ability to conduct interviews and to analyze facts and to exercise sound judgement. Ability to communicate ideas clearly, concisely, orally and in writing. Ability to establish and maintain effective working relationships with staff, clients, and the general public.

Education and Experience:

Any combination of education and experience equivalent to graduation from an accredited college or university with major course work in sociology, social work, human services, or related field. Prefer 2 years experience in related human service work.

Physical Requirements:

• Prolonged periods sitting at a desk and working on a computer.
• Must be able to lift up to 15 pounds at times.
